Problems with the progress report

Hello everyone!  Since a few weeks ago I have problems with the progress report. It shows me a mix between  the learning plan and the progress report. 

Like in the picture:

The problem is when I write the information is duplicated. It's very weird. 

Does something like this happen to someone else?
